Students with a disability who would like to request the accommodation of an Emotional Support Animal must do the following prior to the animal moving in:
Complete the Initial Inquiry Form
The student begins the ESA request process by completing the Disability/Medical Accommodation Inquiry Form. This form notifies HRC of the student’s request and provides basic information about the student’s disability and the animal.
Submit the ESA Request Form
The student and medical/clinical provider complete and submit the Emotional Support Animal Request Form.
HRC Review
The Housing Accommodation Committee reviews the submitted information and documentation.
The student will be notified if the request is conditionally approved, if they need more information, or if it is denied.
If Conditionally Approved, Submit Up-to-date Vet Records and a Photo
If the ESA request is conditionally approved, the student must provide up-to-date veterinary records and a current photo of the animal.
Sign the Assistance Animal Agreement
Prior to the dog moving in, the student must meet with the Housing Accommodation Specialist to review and sign the Assistance Animal Agreement.
Receive a Confirmation Email
Once the ESA is approved, the student will receive an email from the Housing Accommodation Specialist confirming the approval along with any specific instructions.
