Steps to Request a Service Animal

We request that students with a disability who have a service animal do the following prior to the service animal moving in:

  1. Complete the Initial Inquiry Form

    The student begins the process by completing the Disability/Medical Accommodation Inquiry Form. This form notifies HRC of the student’s request and provides basic information about the student’s disability and the animal.

  2. Submit Up-to-date Vet Records and Photo

    The student must provide up-to-date veterinary records and a current photo of the animal.

  3. Sign the Assistance Animal Agreement

    Prior to the dog moving in, the student must meet with the Housing Accommodation Specialist to discuss the appropriate questions related to service animals and sign the Assistance Animal Agreement.

  4. Receive a Confirmation Email

    Once the process is complete, the student will receive an email from the Housing Accommodation Specialist confirming the approval along with any specific instructions.
